Updated CD of PDF Fill-in Forms: Disability & General Membership

Summary

PERAC Memorandum #41 (2009) transmits an updated CD (dated September 2009) containing fill-in PDF forms for Disability and General Membership applications, replacing the March 2009 CD. The only change is the revised "Application for Withdrawal of Accumulated Total Deductions" form (previously announced in Memo #37); all other forms remain unchanged. Boards should begin using the updated Withdrawal form and retain/distribute the new CD accordingly.

Connarton, Executive Director RE: Updated CD of PDF Fill-in Forms: Disability & General Membership DATE: October 26, 2009 As you were advised in PERAC Memorandum #37 of 2009, PERAC has issued an updated form entitled Application for Withdrawal of Accumulated Total Deductions. The attached CD (labeled September 2009) includes a PDF of the updated Withdrawal form with fill-in functionality. All of the other PDFs of forms on the attached CD are just as they were on the CD (labeled PERAC 2009) which was distributed by PERAC in March of 2009.
