TO: All Retirement Boards

RE: Proposed Regulation on Member’s Physician Statement and Workshop on IRS Submissions
DATE: October 8, 2008
Enclosed is a draft of a proposed amendment to the regulation dealing with applications for disability retirement (840
CMR 10.06). The amendment will require applicants for disability retirement to file two Physician’s Statements
instead of one which is required now.
Hearings on these regulations will be conducted at the times and places set forth below. Public comments will be
accepted through November 21, 2008. Directions to the hearing sites can be obtained at
On the same date at the places listed below, PERAC staff members will be available to answer questions and assist in
completing the IRS Form 5300.
