This memo follows up on Memo #28/2003, reminding boards that PERAC's Disclosure Form must be completed and on file for all currently employed investment managers, consultants, and third-party marketers. Boards are required to collect any completed Disclosure Forms not yet submitted to PERAC and forward them to Rose Cipriani in the Investment Unit.

To: All Retirement Boards

Subject: Submission of Disclosure Forms
The new PERAC Disclosure Form that was distributed last summer has proven very helpful in illuminating possible financial relationships involving investment managers, investment consultants, and third party marketers.
In addition to mandating the use of these forms in all subsequent investment manager searches, PERAC Memo #28/2003, dated August 23, 2003, which accompanied the release of the new investment forms, instructed boards “to forward the new Disclosure Form to all existing vendors for filing with the Board.”
At this time, in order to assure that we have updated Disclosure Forms on file for all managers currently employed by Massachusetts public retirement systems, we are asking that boards forward to PERAC all completed Disclosure Forms that have not been previously submitted to us. These forms should be sent to Rose Cipriani in the Investment Unit.
