This memo announces that PERAC has mailed the 2003 Annual Statement of Earned Income (91A) forms directly to disability retirees, with instructions, and that completed forms with all tax documents must be returned to PERAC by April 15, 2004 (mail or hand delivery only—no faxes). Boards should only provide duplicate forms in extreme circumstances (e.g., lost or never-received originals), ensuring member information is properly entered, and may direct members to the accompanying Earned Income Worksheet (also on PERAC's website) to help calculate allowable earnings while on disability.

M E M O R A N D U M TO: All Retirement Board Administrators FROM: Kevin P. Blanchette, Deputy Executive Director RE: 2003 Annual Statement of Earned Income (91A)
DATE: March 2, 2004
Recently, a 2003 Annual Statement of Earned Income (91A) was mailed to each disability retiree, along with the instructions for its completion, copies of which are enclosed. You may make additional copies of the form to assist a member, however, this should only be done in extreme cases, such as when a member may have mistakenly destroyed the original, or it was never received in the first place. It should not be used as a casual substitute for the original form, as PERAC cannot handle the task of accepting duplicate statements. If you need to help a member, please make sure the member’s information is inserted in the blank spaces at the top of page one. The completed statements, with any/all related tax documents attached, are to be returned to PERAC on or before April 15, 2004. Please note that we will not accept faxed copies - they must be mailed or hand delivered to this office.
Also enclosed, please find a copy of the “Earned Income Worksheet” that was mailed out earlier this year to disability retirees who had retired within the last three years. The intent of the worksheet is to assist members in calculating what they can earn if they choose to work while on disability, without exceeding the earnings limit. This information is also available on the PERAC website: www.state.ma.us/perac.
