PERAC Memo #44/2000 corrects a distribution issue with the double-sided Affidavit of Eligibility form (Attachment B to Memo #40/2000) related to the EEOC v. PERAC MEMO #44/2000 M E M O R A N D U M TO: All Retirement Boards FROM: Joseph E. Connarton, Executive Director RE: Affidavit for EEOC Settlement DATE: October 4, 2000 It has come to our attention that some boards did not receive a copy of both sides of the Affidavit of Eligibility to Apply for Retroactive Accidental Disability Retirement Benefits Pursuant to the Settlement of EEOC v. Commonwealth of Massachusetts et al. which was Attachment B with PERAC Memo #40/2000. As a result, we are sending a double-sided copy of the form to all boards. Please also assure that when this form is returned to the board by the applicant, the applicant is given a photocopy of both sides of the form for his or her records.
