PERAC MEMO #45/1999 M E M O R A N D U M TO: All Retirement Boards FROM: Joseph E. Connarton, Executive Director RE: Disabled Retiree Data DATE: December 2, 1999 In order to update the disability database utilized by PERAC to process the 1999 Annual Statements of Earned Income, retirement boards must submit basic information as of December 31, 1999, relative to each accidental and ordinary disability retiree. The data that PERAC currently has on file pertaining to all retirees should be the most updated and accurate. However, we are asking you to review the attached list, and make any status changes, whether it be that the member has passed away, resides in a nursing home, has recently waived his/her allowance or has returned to active service. Also, please add all new retirees who were approved in 1999 for accidental or ordinary disability. Enclosed for your convenience is a form to be utilized to add the new members. Please complete the form in full, and make additional copies if more are needed. It is imperative that all information be returned to PERAC by January 14, 2000, in order for the statements to be mailed to members in a timely manner. Once again PERAC will be sending out a reminder notice to all disability retirees in early January. The intention of this mailing is to notify the members that they will once again receive the Annual Statement of Earned Income Form, and that it is due back to PERAC by April 15, 2000. No additional notices will be sent before we notify you that they have not complied, and their allowances shall cease pursuant to G.L. c. 32, §91A. A copy of the Annual Statement of Earned Income Form and related instructions will be shared with retirement board personnel so that you can become familiar with information PERAC will mail to the disabled retirees.
